It is unknown whether hospital-based medical professionals in Spain educate patients about advance directives. The objective of this research was to determine the frequency of hospital-based physicians’ and nurses’ engagement in AD discussions in the hospital and which patient populations merit such efforts. A short question-and-answer-based survey of physicians and nurses taking care of inpatients was conducted at a university hospital in Madrid, Spain. In total, 283 surveys were collected from medical professionals, of whom 71 per cent were female, with (...) an average age of thirty-four years. Eighty-four per cent had never educated patients about ADs because of lack of perceived responsibility, time, or general knowledge of ADs. Patient populations that warranted AD discussions included those with terminal illnesses, chronic diseases, and elderly patients. Regarding degree of AD understanding in medical professionals: 57 per cent of medical professionals claimed sufficient general knowledge of ADs, 19 per cent understood particulars regarding AD document creation, and 16 per cent were aware of AD regulatory policies. Engagement in AD discussions was considered important by 83 per cent of medical professionals, with 79 per cent interested in participating in such discussions themselves. The majority of hospital physicians and nurses do not educate their patients about ADs, despite acknowledging their importance. Patient populations of highest priority included those with terminal diseases or chronic illness or who are of advanced age. (shrink)

Taking as its starting point significant similarities between a formal language model—Grammar Systems—and a grammatical theory—Autolexical Syntax—in this paper we suggest the application of the former to the topic of the latter. To show the applicability of Grammar Systems Theory to grammatical description, we introduce a formal-language-theoretic framework for the architecture of natural language grammar: Linguistic Grammar Systems. Nowadays, there is a quite considerable amount of literature on the use of analogy or more generally of inferences by parallel reasoning in contemporary legal reasoning and particularly so within Common Law. These studies are often motivated by research in artificial intelligence seeking to develop suitable software-support for legal reasoning. Recently, Rahman et al. developed a dialogical approach in the framework of Constructive Type Theory to what in Islamic Jurisprudence was called qiyās or correlational inferences. In their last chapter the (...) authors suggested that such an approach contributes to the study of patterns of reasoning by precedent cases within contemporary Common Law. In the present paper we will further motivate the deployment of the dialogical framework developed in Rahman et al. within Civil and Common Law. After a presentation of Scott Brewer’s take on analogy within Common Law, that has striking structural similarities to reasoning by precedent case rooted in ratio legis, we will illustrate the implementation of the framework with a brief discussion of some cases of legal reasoning based on Spanish Civil Law but where the accent is put in the emerging ruling rather than in the existing of a case in Common Law. Moreover, quite surprisingly, the case under study suggests that even cases of Law interpretation fit the argumentation pattern of qiyās al-‘illa. A caveat: in the present paper we will focus mainly in discussing the dynamics of the meaning constitution involved rather than in setting the rules of the underlying dialogical framework, the latter is the subject of a follow-up paper. Digital transformation has allowed to offer additional services - which complement the main product - both in terms of use, emotional and relationship terms. Focused on a traditionally rational insurance customer offering a value that explores the customer's emotions, from co-creating with the user, allows brand differentiation. Given this idea, this document has three purposes. First, identify the role of expectations and the perceived quality of the customer's digital experience. Secondly, to identify the relationship between experience and satisfaction gained in (...) private insurance web areas. Third, identify the most valued digital attributes by the user. A sample of 4,178 customers registered was analyzed using the partial least squares technique. The model is highly predictive to customer experience and evidence the important relationship between the WOW effect and satisfaction, as well as the weak but doble role that expectations play on insurance digital self-service. The model demonstrates that expectations are only relevant before web consumption, because during the process it is the perceived digital quality, in particular the usefulness, information and technology, that offer a true customer experience. This article offers high academic value because it more accurately defines the determinants of the digital insurance customer experience and its effect on customer satisfaction in digital services. While expectations influence attitude before service, this research reveals that perceived digital quality is what delivers a true customer experience. Strategically, the implications are immediate in the field of business as it shows the importance of co-creation in digital design, not only because of the significant savings in implementation costs but also because it guarantees a greater experience, essential in the loyalty of its customers. Cognitive Science is a promising field of research that deals with one of the most fundamental questions ever: how do beings know? However, despite the long and extensive tradition of the field it has not yet become an area of knowledge with scientific identity. This is primarily due to three reasons: the lack of boundaries in defining the object of study, i.e. cognition, the lack of a precise, robust and consistent scientific methodology and results, and the inner problems derived from (...) its interdisciplinary nature. ObjectiveIntimate partner violence has been related to emotional dependence and addictive disorders. This study aims to provide a global approach to analyse the relationship between these variables and to determine the factors underlying permanence in violent relationships.MethodsIt is a non-experimental, cross-sectional correlational design study. Participants had to have at least one dating relationship for at least 1 month to complete the questionnaire, which included the following instruments: emotional dependence scale, scale of violence in dating relationships and impulse control disorders scale.ResultsThe (...) sample consisted of 1.533 adolescents, 53.9% were male and 46.1% female, between 14 and 18 years. Emotional dependence correlated significantly with received violence. Compulsive spending is strongly associated with emotional dependence, whereas sex addiction is associated with received violence to a greater extent. Received violence mediates on emotional dependence and addictions.ConclusionIPV is a risk factor for the perpetuation of addictive behaviours. It is advisable to promote affective education for developing resilience and adequate coping. (shrink)

Epidemiological evidence supports a health‐promoting effect of the Mediterranean Diet (MedDiet), especially in the prevention of cardiovascular diseases. These cardiovascular benefits have been attributed to a number of components of the MedDiet such as monounsaturated fatty acids, antioxidant vitamins and phytochemicals. However, the underlying mechanisms remain unknown. Likewise, little is known about the genes that define inter‐individual variation in response to the MedDiet, although the TCF7L2 gene is emerging as an illustrative candidate for determining relative risk of cardiovascular events in (...) response to the MedDiet. Moreover, omics technologies are providing evidence supporting potential mechanisms, some of them implicating epigenetics (i.e. microRNAs, methylation), and certain data suggest that some traditional foods could contribute via microRNAs possibly acting as exogenous regulators of gene expression. Future research should aim at increasing and consolidating the nutrigenetic and nutrigenomic knowledge of the MedDiet in order to provide sound, personalized and optimized nutritional recommendations. The present study aims to explore whether the symptoms associated with fibromyalgia are contextually influenced by the area of residence. Furthermore, it is analyzed whether the effect of the acceptance of the disease on the emotional, cognitive and physical symptoms is moderated by the patients’ place of residence. Using a cross-sectional design, a total of 234 women with fibromyalgia were surveyed, of which 55.13% resided in rural areas and 44.87% in urban areas. Self-reported questionnaires were used to assess pain severity, (...) anxiety and depression, functional limitation, physical and mental fatigue and acceptance of the disease. The results show significant differences in acceptance, pain, and physical and mental fatigue between patients from rural and urban areas. The rural area patients presented higher levels of acceptance and pain and lesser levels of physical and mental fatigue compared to the urban area. The moderation analysis add that, only in patients from the rural area, the variables of physical symptoms were significantly and negatively associated with acceptance. This study addresses for the first time the role of the place of residence in suffering from fibromyalgia, suggesting that the rural or urban environment plays a relevant role in the severity and/or management of symptoms in fibromyalgia women. Este trabajo se inscribe en una revisión in progress del Derecho antidiscriminatorio moderno que las autoras vienen efectuando desde hace más de tres lustros. En la primera parte del mismo se examinan los textos de una f ilósofa política y de dos juristas esta- dounidenses (I. M. Young, C. A. MacKinnon y K. Crensahw) que inspiran la propuesta de un concepto de discriminación y de una visión de la interseccionalidad basados en el reconocimiento de los sistemas de opresión. La segunda (...) parte se centra en el análisis del concepto de discriminación múltiple o interseccional propuesto por la doctrina jurídica española a partir de la jurisprudencia referida al caso de una mujer (“La Nena”) casada por el rito gitano que ve denegada su pensión de viudedad. A partir de lo anterior, el trabajo concluye con una serie de propuestas entre las que destacan las referidas a la necesidad de evitar que el discurso sobre la interseccionalidad disuelva la identidad política de los grupos en la individualidad, refuerce el sistema sexo-género y consolide un Derecho antidiscriminatorio de corte excesivamente jurisprudencial. Algunos corresponden a vocablos simples, como son, por ejemplo, Alfás y Haza; en otros, el resultado romance de faḥṣ aparece como primero o segundo elemento de vocablos compuestos, cual es el caso de Fazhaliy Moratalaz Con este trabajo pretendemos explicar el origen y significados de algunos topónimos hispanos y al mismo tiempo tratamos de corroborar la tesis previamente expuesta de que el nombre común haza procede (...) del árabe faḥṣ 'campo' y no del latín fascia 'banda, tira'. Este artículo aporta una clarificación sobre lo que sí podemos decir de la relación entre este mundo y el futuro anunciado por la esperanza cristiana (escatología). La iluminación según la sagrada Escritura de la recíproca interdependencia y complementariedad entre el futuro intrahistórico y el metahistórico, permite encontrar pautas teórico-prácticas de actuación. Una humanidad nueva conduce a forjar un mundo según el plan de Dios, un mundo donde ha de reinar la paz y la justicia. El futuro es la ciudad de (...) la Paz; construirla es tarea de todos. En esta hora de la historia, la mujer está dando una aportación digna de consideración. El sueño de futuro que alimenta el Apocalipsis no es una ilusión, pues se está gestando ya en lo escondido de la historia. (shrink)

En este artículo tratamos de demostrar que el término español haza procede del árabe Faḥṣ 'campo' y no del latín Fascia 'banda, tira', tesis admitida de manera unánime. Para conseguir nuestro propósito hemos reconstruido la historia de este vocablo a través del estudio de fuentes documentales y textos literarios. Como resultado de ello demostramos que haza, en su forma arcaica faza, aparece en Castilla en el siglo IX; ponemos de manifiesto que en un principio asumió los significados del étimo árabe, (...) para desarrollar más tarde nuevos sentidos y finalmente, explicamos la evolución fonética experimentada por faḥṣ hasta convertirse en haza. Es preciso tomar conciencia del (...) Espíritu presente en nuestra interioridad. Religiosas/os han encarnado la fertilidad del Espíritu y la ilustran para nuestro tiempo. En este artículo tratamos de abordar un problema de difícil solución: la forma de tra-ducir el término mawlá, cuyo significado no es el mismo en todas las regiones y etapas históricas. Estudiamos el uso que de este vocablo y de su plural se hizo en al-Andalus desde la época de la conquista hasta la caída de califato de Córdoba, sumando observa-ciones personales relativas a los tipos de walaŽ que se establecieron, al papel desempe-ñado por el grupo social denominado "los mawali" (...) y a factores muy diversos que ayu-dan a interpretar correctamente dicho nombre. ABSTRACT In this paper we address a problem with no easy solution: the translation of the term mawlá, whose meaning differs from region to region and is subject to change over time. We study the use of this word in al-Andalus from the period of the Conquest to the fall of the Caliphate of Cordova, adding some personal observations concerning the types of wald', the role played by the social group called "the mawali" and many other factors that help to establish the correct interpretation of this word.
